The History office is asking members to fill out a
Biographical and Weather questionaire to compile as much information
as they can. The forms are "Fillable", you can type in your
information in the boxes next to or just below the question.
When you have completed the form, you can print it out for your
personal copy. Please save the form as a file on your computer
and e-mail the completed forms to the History office at:
